Records Department and Forms

All forms are available at the Santa Fe Police Department.

Quality Service Forms (commendations or complaints) Can be picked up at the Dispatch office at the Police Department. All forms need to be returned to the Chief of Police, Walter Braun or emailed to records@santafetx.gov. These forms can be mailed to the above address, emailed to the email address listed or can be returned to the police department in a sealed envelope.

The Golf Cart registration form can be found at the bottom of the screen.

Records Request

Open Records Request Form
This form can be obtained at the Santa Fe Police Department Records Office or Dispatch Office. Once the form is completed, please return it to the Records Department.  Open record requests can also be faxed to 409-925-4806.
